OP updated with the latest sigpatches for Atmosphere 1.2.6 and Firmware 13.2.1. I noticed that the latest pack doesn't include any ES or FS patches for the firmware update, seems like only those for Atmosphere 1.2.6 were added. I suppose Nintendo didn't really make much changes so the ones from back in 13.1.0 still work lol

First off, update to Atmosphere 1.4.0, same for the signature patches I just added to the OP. If after updating both Atmosphere and the sigpatches you still have that issue, then try booting in Maintenance mode. It's absurd how many issues that simple thing that fix.

I updated Atmosphere, hekate, and sigpatches, as soon as the latest hekate came out. No issues With 16.0.3. Didn't even need to clear/retrain my mod chip. Also ran Lockpic_RCM and dumped keys, but there doesn't appear to be any new keys. Not sure what this update does at all, but I'm sure the latest ToTK patch runs without it.
